Carruth Data Breach

On January 13, 2025, the District was informed by Carruth Compliance Consulting, Inc. of a data security incident that may impact all David Douglas School District (DDSD) employees. Carruth Compliance Consulting is a third-party administrator overseeing compliance for 403(b) and 457(b) retirement plans for many Oregon school districts, including ours.

Because the District securely provides Carruth with employee information—including names, Social Security numbers, home addresses, and hire dates—to facilitate plan administration and employee advisement, all DDSD employees are potentially affected by this incident.

What Information Was Potentially Affected?


The compromised data may include:

  • Name
  • Social Security number
  • Financial account information

In some cases, additional information may have been exposed, such as:

Tax filings

Driver’s license number

W-2 data

Medical billing information (excluding medical records)

Monitor Your Financial Accounts

Regularly review your bank, credit card, and financial accounts for any suspicious activity and report unauthorized transactions to your financial institution immediately.

Check Your Credit Reports

You are entitled to a free annual credit report from each of the three major credit bureaus at www.annualcreditreport.com or by calling 1-877-322-8228.

Consider a Fraud Alert or Credit Freeze

  • A fraud alert requests that creditors verify your identity before issuing new credit.
  • A credit freeze restricts access to your credit report, preventing new credit accounts from being opened in your name.

Report Any Suspicious Activity

If you suspect identity theft, report it to the Federal Trade Commission (FTC) at www.identitytheft.gov or call 1-877-ID-THEFT (1-877-438-4338). You may also consider filing a police report.
