COVID-19 Information for MECP Families and Staff

For Staff

MECP Staff needing to enter an MECP office building, ECSE classroom, or speech clinic must fill out this form 24 hours prior to coming in. Your MECP administrator will review this form prior to you being allowed entry into the building. Once inside, all safety protocols must be followed.

COVID Screening Reporting Tool (students): This form should be filled out by MECP staff for any students in the MECP program who have tested positive for COVID-19, who have a household member who has tested positive for COVID-19, or who have been notified that they are a close contact of a known case of COVID-19.

COVID Screening Reporting Tool (staff): MECP staff members who have tested positive for COVID-19, who have a household member who has tested positive for COVID-19, or who have been notified that they are a close contact for a known case of COVID-19 need to notify their direct administrator immediately. Administrators will ask follow up questions and submit appropriate forms.
